N&D SUPPORT SERVICES

About us

N and d focus on working closely with every team member of each individual. This can include but not limited to, OPA, PBS practitioners, plan nominees, physio therapist, occupational therapist, mainstream supports as well as psychologists. We encourage regular case conferences and keep all stake holders involved each step of the way. Our support team assist with connecting individuals with allied health professionals ( AHP ) to follow support plans and review plans as each individual grows, support plans are reviewed regularly. This helps to maximise the support service, independence and growth of individuals and support staff.

Person Centred Approach

At every step of support process we put the individual first. We work with the person to create the programme best suited to their needs and goals. We display every option available to the person and assist with the pro’s and con’s with the choices available. Which takes into account the physical, emotional, social and spiritual needs of each individual. We do this Together with all stake holders, support networks and us at N and D . then the individual will create the best possible support system and programme to help build on independence and growth, empower the person to live the best possible life and the experiences life has to give as independently as possible.   

We feel working with the individual will always achieve the best possible outcome to independence and growth.

Advance Documentation System

Our Documentation

N and D understands the importance of proper documentation. We how prompt and accurate documentation is vital to an individual’s development and growth. We understand an individual’s circumstances can change at any point in their life or their skill set has changed, and skill criteria needs adjusting. This is where accurate documentation comes in handy for care plan review and were new identified skill learning opportunities arise. We encourage the individual to be in charge of his supports at every stage in their plan. We strongly believe the individual is the best person to make decisions regarding their care needs and goals. 

N and D have a system that files documentation which complies with the NDIS plan review process. We make information easy to access while also respecting the confidentiality and privacy of all individuals. We at every point ask for consent to share any information to relevant stake holders and family.

Staff Recruitment

  • Unique Recruitment Process

    We have a very unique recruitment process, we look for and train highly motivated and compassionate staff, we at N and D feel that finding compassionate staff who want to achieve the best results for the individual and the growth of N and D and of course themselves, will grow with the individual. Our staff are trained in a person-centred approach, duty of care, dignity of risk, we are always looking for opportunities to up skill staff to better support the people with disabilities and to communicate with all people involved.

  • Well Trained Staff

    Staff are trained to understand each individual is different and has different needs, goals, support needs. Staff are trained to identify changes in life circumstances and to identify any risks through an assessment process.

  • Assessing Qualifications

    Our recruitment process begins with an initial screening of applications, where we assess qualifications, experience and suitability for the role. We only consider applicants who have relevant qualifications and experience in working with people with disabilities. We also highly regard compassionate applicants.

  • Interview Process

    We then conduct a thorough background check before shortlisting candidates. We then run an interview process face to face to better understand the applicant and any other skills they may have.

  • Suitable Placement

    After interviewing, we finalise selections and find suitable placement that will enhance individuals and themselves. We strongly believe everyone can teach someone something in life skills, being support staff or people.
