How to Refer

To refer someone to our services. Simply complete the referral form on this page, the form requires basic information about the individual, including their name, contact details, and the support they require. Once we receive the referral, we will contact the individual or relevant stake holders to discuss the support needs and goals and arrange a consultation.  

  • Confidentiality

    All information provided will be treated as confidential and will only be used for the purpose of providing support services.

  • Support planning

    Once the referral has been processed, our team will work with the individual and their support network to develop a tailored support plan that meets their unique needs and goals. We tailor our supports to each individual. Feel free to contact us with any questions.
