N and D support understand that nursing care is a vital need to an individual’s care, we recognise the needs for people with disabilities and structure a plan that is set to each individual to manage their health and well-being. We make sure every individual has accessed the absolute best care possible.  

• PROVIDING NURSING CARE. We work with each individual and assist with medication administration, monitor vital signs, we encourage the individual to work with staff to help monitor and document, provide wound care, we have experienced and compassionate staff to provide the best possible person-centred care. 
• SUPPPORT WITH HEALTH MANAGEMENT; we support each individual to understand the diagnoses and any management plan that is in place. We support the individual to access information, understand the monitoring process and documentation, we assist with reviews and the change of circumstances for the individual. 
• ASSISTING THE INDIVIDUAL TO ACCES HEALTH CARE SERVICES; including scheduling, appointments, transportation, accompanying and translating the doctor’s advice and hard to process information if needed. 
• PROVIDE EDUCATION AND TRAINING; we provide ongoing support and reviews annually for the individual and their families. We train the support staff for each individual and all stakeholders to ensure they have the knowledge and skills they need to support and manage our customers health and well-being. 
• PROVIDE ONGOING SUPPORT AND REVIEW; we provide ongoing support and review of individuals progress and put in place any necessary adjustments to their care plan as needed and explain or demonstrate the process to the individual at every step.
